COURSE EVALUATION
Skill evaluations – Skills must be satisfactorily completed in the following areas:

B. Treatment area/unit preparation setup (1)/breakdown (1) 2

F. Respiratory, pulse and temperature 1
G. Extraoral/Intraoral (1), Hard Tissue Charting (HTC) (1) 2
H. Medical histories (MdHx) 2
I. Instrument transfer between operator and assistant 1
J. Mock appointment MdHx through HTC (5 comps) 1
Each skill evaluation is worth 4 points.
Skill evaluation will be completed by random selection during lab time. Being unprepared to complete competency if selected will result in a 1 point deduction.
Any “U” (Unsatisfactory) results in 0 points.
Before you may progress to the next skill: Any skill evaluation earning 1 (one) or 0 (zero)
points must be repeated until completed satisfactorily.
Handwashing, Autoclave, Blood pressure, and Vital sign Skill Evaluations
4 points = All Satisfactory (S)
3 points = No more than 1 improvement needed (I)
2 points = No more than 2 “I’s”
1 point = No more than 3 “I’s”
0 points = 4 or more “I’s” or any “U”
All Other Skill Evaluation:
4 points = All Satisfactory (S) or no more than 1 improvement needed (I)
3 points = No more than 2 “I’s”
2 points = No more than 3 “I’s”
1 point = No more than 4 “I’s”
0 points = Any Unsatisfactory (U)
Any points received for Unprofessional behavior will be taken from your total points accumulated.
MOCK PATIENT
This skill evaluation will consist of student/teams performing clinical assessment procedures on student partner “patients.” It will be the culmination of clinical skills learned throughout the
semester. The mock patient will be graded as a whole. The mock patient will be worth 20 points. Any “I” will be a 1 point deduction. Any “U” will be a 3 point deduction. There will be no part of the evaluation repeated even if a U is received.
GRADING SCALE:
Skill Evaluations 14 x 4 points = 56 points
Mock Patient 5 competencies x 4 points = 20 points
Lab quizzes 6 X 4 points = 24 points
Dental care.com assn. 3 X 5 points = 15 points
Clinical Observation = 15 points

CLASSROOM POLICIES
A. Students should arrive 10 minutes prior to the scheduled start time of their lab to set up and prepare for laboratory activities.
B. Absences and Make-up Work
1. The student is expected to attend each laboratory.
2. The student is responsible for all material covered or assigned during classes
or labs.
3. In case of absences the instructor should be notified (in advance, if possible).
4. Absences must be excused in order to not lose points (1 point per unexcused
absences and .25 points per tardy).
C. Academic Honesty
Each student is expected to be in complete compliance with the college policy on
academic honesty as set forth in the college catalog and the student handbook. Any
student cheating on an exam will receive a zero on the exam which cannot be replaced by
the final exam score and the professor at his discretion can withdraw the student from the
class.

D. Withdrawal
Per Valencia Policy 4-07 (Academic Progress, Course Attendance and Grades, and
withdrawals), a student who withdraws from class before the withdrawal deadline of
November 10, 2017 will receive a grade of “W.” A student is not permitted to withdraw
after the withdrawal deadline. A faculty member MAY withdraw a student up to the
beginning of the final exam period for violation of the class attendance policy. A student
who is withdrawn by faculty for violation of the class attendance policy will receive a
grade of “W.” Any student who withdraws or is withdrawn from a class during a third or
subsequent attempt in the same course will be assigned a grade of “F.” For a complete

E. Electronic Devices
We ask that there be no cell phones in lab. This is a source of contamination and is not permitted in the dental hygiene clinic.
The exception would be for an emergency situation that was explained to the professor
before class. Points may be deducted at the discretion of the instructor.
F. “Students with disabilities who qualify for academic accommodations must provide a
